Office Policies
Cancellation Policy
At Orthopaedic Associates, we do our best to schedule your appointments in a timely manner. In order to help us, we ask that you notify our office 24 hours prior to your scheduled appointment if you must cancel for any reason. This will allow us to schedule other patients in that time slot. This means they are able to get in and see one of our physicians. It is our policy to charge a $25 no show/cancel fee to any patient who does not cancel their appointment within this time period. When you sign in for the first time in our office, you will sign a form confirming that you understand this policy.
Same Day Appointments
Please note that if your appointment is scheduled on the same day you are referred to us, you may have to wait longer than usual, as we are working you into our schedule. We appreciate your patience.
Patient Records
In order to maintain accuracy in your patient record, we require that you give our office current information on each and every visit. This includes your name, changes to your address and telephone number, changes in your health status and information about other health services that you may have received, and your current insurance information.
Disability Forms
If you have a disability form and/or a FMLA form to be completed, please allow 5-10 working days for completion. Due to the high volume of forms that come in, we charge a fee of $10 that must be paid prior to form completion.
X Rays
If you need a copy of your x-rays, please call us at least three days prior to picking them up to ensure that they are ready for you. When you call to request copies of x-rays, press option “5”. We are required to maintain your original x-ray films in our office. Therefore, if you need your x-rays, we will make a copy of the firms for you. A payment of $10 per film will be required at the time you pick up your x-rays for their release.
Refund Policy
If you are due a refund on your account and you have not received payment in a timely fashion, please call our office and ask to speak to the billing department. We regularly issue refunds to patients, however, if you have not received your refund, please call to ensure we have your account posted correctly. We appreciate your patience with this process.
